Staff/Leader-Student Communication Permission Form

Staff/Leaders at Fairview Christian Church are encouraged to communicate with students for discipleship purposes using a variety of effective methods, including electronic communication. When authorized to do so, staff/leaders may use their personal phone numbers, social media accounts etc. to contact students for the purpose of encouragement, discipleship, following up, checking in, or reminding of group activities. We ask that you fill out your information, your student's information, and select the appropriate permission settings below.
